I've only seen this on Windows 2000 and XP. This happens every so often when I'm in the middle of a debugging a program, possibly looking at code or something else other than menus. When I look at the menu bar, it is blank. The bar is there but all the menus have disappeared.

Any idea what causes it to happen? They come back if I logout and log back in.
